Abstract

Introduction

Basal cell carcinoma and squamous cell carcinoma are among the most common cutaneous neoplasms in elderly patients. The presence of both lesions within the same anatomical site is non very common and is known as a collision tumor. We present a collision tumor involving the auricle and discuss the diagnostic challenges as well as considerations for its surgical and medical management.

Case Report

A 78-year-old male patient presented with a lesion located on the antihelix. Primary excision was performed, and histopathological examination revealed positive surgical margins. A second excision was subsequently made. Histopathological analysis demonstrated the presence of an intraepithelial focus consistent with basal cell carcinoma and a separate independent focus of squamous cell carcinoma, both on a background of chronic actinic damage. Due to the coexistence of these two neoplasms within the same anatomical region, a diagnosis of collision tumor was established. Surgical resection achieved local disease control. The role of adjuvant medical therapy and its importance in providing comprehensive patient management are also discussed.

Discussion

Collision tumors represent an uncommon entity among cutaneous neoplasms. Definitive diagnosis is established through histopathological examination of the surgical specimen. In the present case, the auricular location added complexity to treatment planning. The auricle possesses a complex anatomy and offers limited tissue availability for reconstruction. Although surgical excision and margin determination must follow oncologic principles, reconstruction must also remain deasible. Achieving adequate oncologic margins while avoiding significant deformity constitutes a surgical challenge in the management of these lesions.

Conclusion

Collision tumors are rare entities that require a high index of suspicion among surgeons, clinicians, and pathologists. Correct surgical excision remains its treatment. Histopathological confirmation of complete excision should always be obtained through appropriate postoperative pathological assessment.
